Permission Slips

 All students must have a signed permission slip on file in the library in order to check out materials from the library.  There are different permission slips for different grades.  Click on the appropriate link below to print and sign the required slip.

 

K-4 students have the opportunity to check out CROW Backpacks from the elementary library.  CROW stands for Connecting Reading With the Outside World.  This iniative was created by Dr. Yard, the science teachers and the enrichment teachers.  Each backpack investigates a different aspect of nature while encouraging a love of reading.  In order to check out a backpack, students must have parental/guardian permission.  Below is a link to the necessary slip:

  

Young Adult Materials
Students must have parental or guardian permission to check out Young Adult materials. Materials considered “Young Adult” are intended for mature readers age 12 – 18. YA is a broad category, which may cover the exploration of problems and concerns of teenagers (e.g. drugs, relationships, etc.), portrayals of realistic violence, mature humor, as well as horror fiction.  Young Adult works often discuss topics that are controversial in nature. They can be powerful learning and coping tools, but may be inappropriate for certain students. If you would like your child to be able to check these materials out, please print, sign, and return to school the slip accessible from the link below. You can always give permission on a book by book basis as well.
